Integrated Care Pathway

 

The Integrated Care Pathway (ICP) is a fully funded ACC rehabilitation programme for serious knee, shoulder, and lower back injuries. This pathway is designed to provide a wraparound level of care to support our Kiritaki (patients) through their rehabilitation journey, so they can get back to doing the things they love after an injury.

Active Health New Zealand are dedicated providers of the Integrated Care Pathway, working with valued partners across our network to provide the best care possible.

Kiritaki on the ICP with Active Health are provided with access to:

  • Specialist and surgical input
  • Pre and post-op physiotherapy
  • Cultural support for both kiritaki and treatment providers
  • Strength and return to sport testing
  • Return to work support
  • Other allied health professionals you may need to reach your goals

What happens on the ICP with Active Health?

Kiritaki with specific knee, shoulder, or lower back injuries are referred to Active Health for a Triage assessment with an experienced senior physiotherapist. As part of this process, we can access any imaging or medical input required to ensure we are supporting you and your injury in the best way possible.

Upon confirmation of your diagnosis and acceptance into the Integrated Care Pathway, we will discuss a plan and commence your rehabilitation journey. This may involve a surgical opinion, return to work support, or gym funding.

Throughout your rehabilitation journey, we are here to support you in all aspects. A dedicated ICP Navigator will be assigned to you to help as you navigate your way to achieving your goals.
